ABSTRACT

Background: As Anatomical education advances very rapidly, and at the same time, there is huge demand and scarcity of true biological models, mainly brain tissue. In this scenario, in 1977, Guther Von Hagens worked on plastics and experimented voraciously on diffusing various plastics into large specimens and ultimately succeeded and coined the term “Plastination”. This technique is very popular in Western countries as the latter greatly minimizes the health hazards due to formalin exposure while dealing with biological tissue. In continuation with the advancement of expensive plastination to cost-effective resin casting, especially studies over the complex structures like the brain and spinal cord. In This study, we emphasize the cost-effective Resin cast-insitu method to demonstrate the vasculature of the brain and spinal cord. Materials and Methods: After obtaining the prior permission, we collected the Five (5) Goat heads from the slaughterhouse. Following the standard dissection procedure over the neck region, identified and canulated the major neck vessels. Through the vessels normal saline water, formalin, and pigmented resin were administered and preserved the goat head using routine preservation technique. After 36 to 48 hours, the routine dissection was scheduled to expose the brain and upper spinal cord segments, and finding were captured and recorded. Results and Discussion: The specimens show good penetration of dye in the artery and veins, and it’s easy to appreciate and study the vasculature of the brain and upper spinal cord segments, including the Bastons plexus of veins. Conclusion: In comparison with regular silicon casting, resin casting is very cost-effective and long-lasting, with good penetration of the resin substance up to the capillary level. Furthermore, similar studies may be conducted using in combination with whole organ plastination using silicon and resin embedding.

ABSTRACT

Objective: There is limited evidence about the cross-cultural validity of autistic symptoms in school-aged children in Brazil. We used data from a large school survey to evaluate the factor structure of autism symptoms in community-dwelling children and adolescents. Methods: We translated the Childhood Autism Spectrum Test to Brazilian Portuguese and performed factor analyses to investigate the factor structure of parent-reported autistic symptoms in a large sample (n=8,571) of children/adolescents from a school survey in the metropolitan area of São Paulo. Results: Autistic symptoms were best conceptualized under a correlated-factors model with two factors: one predominantly characterized by social-communication symptoms and the other by symptoms of inflexible/restricted language, behaviors, and interests. Conclusions: These findings provide evidence that the structure of autistic symptoms in Brazil is similar to that described in other countries, indicating the cross-cultural validity of autism in Brazil.

ABSTRACT

Context: The widely accepted caries risk assessment tools such as the Cariogram model needs modifications in the scoring of different factors for reliable results. Aims: The study was aimed to report the caries experience in 3?60 years of the Rajasthan population and to apply the newly derived average decayed exfoliated filled teeth/Decayed Missing Filled Teeth (deft/DMFT) scores in the Cariogram model to assess the caries risk. Settings and Design: The cross?sectional study was planned in the Department of Dentistry and a total of 500 participants were equally divided into five groups (3?6, 7?12, 13?30, 31?44, and 45?60 years) were included. Methods and Material: All participants were examined for caries detection using deft/DMFT, ICDAS, and CAST indices according to the predefined protocol. A single examiner evaluated the participants in a dental operatory and data was recorded. Statistical Analysis Used: One?Way ANOVA and Tukey’s Post?Hoc tests were used to evaluate the significant difference between the groups of each caries index. Results: A statistically significant difference was observed between the mean deft/ DMFT and CAST scores of 3?6, 7?12, and 13?30 age groups with the 45?60 years age group. Caries prevalence was highest (83%) in 31?44 and 45?60 year age groups and lowest (51%) in the 3?6 year age group. Conclusions: Caries experience scores increase as the age increases and the highest scores were observed in 31?44 and 45?60 year age groups and the lowest were observed in the 3?6 year age group. The average ICDAS and CAST scores were higher when compared to the deft/DMFT index

ABSTRACT

SUMMARY: A comparative study of the morphology of suboccipital cavernous sinus (SCS) using MRI and cast specimens was performed. The present retrospective study analysed the craniocervical magnetic resonance venography (MRV) imaging data of 61 patients. Three-dimensional reconstruction was performed using Mimics 19.0. The SCS left-right diameter(d1), distance from the midline (d2), supero-inferior diameter(d3), anteroposterior diameter (d4), distance from posterior diameter to skin (d5), and diameter of the SCS at different parts (d6-d8) were measured. Comparison between MRV images and cast specimens, the SCS, marginal sinus, anterior condylar vein, and vertebral artery venous plexus were symmetrical and could be bilaterally displayed, whereas the presence of extra condylar vein and posterior condylar vein exhibited different types. The adjacency between the SCS and its communicating vessels and changes in its communicating vessels corresponded well with the MRV images and cast specimens. Many types of the presence of left and right lateral condylar and posterior condylar veins were found in the cast specimens, which could be divided into the bilateral presence of posterior condylar and lateral condylar veins, unilateral presence of posterior condylar veins, and unilateral presence of lateral condylar vein. A total of 61 cases analysed using MRV images revealed the bilateral presence of posterior condylar and lateral condylar veins (77.1 %), the unilateral presence of posterior condylar vein (18.0 %), and the unilateral presence of lateral condylar vein (9.8 %), of which the bilateral presence of posterior condylar and lateral condylar veins accounted for the largest proportion. MRV images and cast specimens of the SCS showed its normal morphological structure and adjacency, thus providing accurate and complete Three-dimensional imaging anatomical data of the SCS and its communicating vascular structures. This study enriches the Chinese SCS imaging anatomy data and may be valuable in clinical practice.


RESUMEN: Se realizó un estudio comparativo de la morfología del seno cavernoso suboccipital (SCS) mediante resonancia magnética y muestras de yeso. El presente estudio retrospectivo analizó los datos de imágenes de venografía por resonancia magnética (RNM) craneocervical de 61 pacientes. La reconstrucción tridimensional se realizó con Mimics 19.0. Se midió: el diámetro izquierdo-derecho del SCS (d1), la distancia desde la línea mediana (d2), el diámetro superoinferior (d3), el diámetro anteroposterior (d4), la distancia desde el diámetro posterior hasta la piel (d5) y el diámetro del SCS en diferentes partes (d6-d8). En la comparación entre las imágenes RNM y las muestras de yeso, el SCS, el seno marginal, la vena condilar anterior y el plexo venoso de la arteria vertebral eran simétricos y se observaron bilateralmente, mientras que la presencia de la vena extracondilar y la vena condilar posterior presentaba tipos diferentes. La proximidad del SCS y sus vasos comunicantes y los cambios en sus vasos comunicantes se correspondieron bien con las imágenes de RNM y los especímenes moldeados. Se encontraron muchos tipos de venas condilares laterales y condilares posteriores izquierda y derecha en las muestras de yeso, que podrían dividirse en presencia bilateral de venas condilares posteriores y condilares laterales, presencia unilateral de venas condilares posteriores y presencia unilateral de venas condilares laterales. Un total de 61 casos analizados mediante imágenes MRV revelaron la presencia bilateral de venas condilares posteriores y condilares laterales (77,1 %), la presencia unilateral de venas condilares posteriores (18,0 %) y la presencia unilateral de venas condilares laterales (9,8 %) de los cuales la presencia bilateral de las venas condilar posterior y condilar lateral representó la mayor proporción. Las imágenes de RNM y las muestras de yeso del SCS mostraron su estructura morfológica y adyacencia normales, lo que proporcionó datos anatómicos de imágenes tridimensionales precisos y completos del SCS y sus estructuras vasculares comunicantes. Este estudio enriquece los datos de anatomía de imágenes de SCS chino y puede ser valioso en la práctica clínica.

ABSTRACT

Objetivo: O objetivo deste estudo foi responder qual é a melhor técnica para remoção de pinos de fibra de vidro e pinos metálicos fundidos considerando tempo, preservação da estrutura dentária e custos. Métodos: Foi realizado um estudo in vitro, randomizado e cego. Sessenta dentes tratados endodonticamente foram randomizados em dois grupos de acordo com o tipo de pino. Uma segunda randomização foi realizada para cada tipo de técnica de remoção (ultrassom, broca ou combinada; n=10). One-way ANOVA foi usado para comparar o tempo de remoção do pino, teste t pareado comparou a quantidade de estrutura dentária removida e Kruskal Wallis seguido pelo teste post hoc de Mann-Whitney foram usados para análise de custo (P = 0,05). Resultados: Não houve diferença no tempo de remoção do pino considerando o tipo ou técnica do pino (P=0,630). A perda média de estrutura dentária na região cervical foi superior a 30% quando apenas uma ponta diamantada foi usada para remover pino de fibra de vidro (P<0,00001) e pino metálico fundido (P=0,008). Conclusão: De acordo com os resultados deste estudo, podemos concluir que a seleção da técnica de remoção dependerá da habilidade do operador, e sempre ocorrerá alguma perda de estrutura dentária, embora seja esperada uma perda maior quando apenas pontas diamantadas são usadas para este fim.(AU)


Objective: The aim of this study was to answer which is the best technique to removing glass fiber post and cast metal post considering time, preservation of tooth structure, and costs. Methods: An in vitro, randomized, blinded study was conducted. Sixty endodontically treated teeth were randomized into two groups according to post type. A second randomization was performed for each type of removal technique (ultrasound, drill, or combined; n=10). One-way ANOVA was used to compare the time to post removal, paired t-test compared the amount of tooth structure removed and Kruskal Wallis followed by post hoc Mann-Whitney test were used for cost analysis (P=0.05). Results: There was no difference in post removal time considering post type or technique (P=0.630). The average loss of tooth structure in the cervical region was greater than 30% when only a diamond bur was used to remove fiber glass post (P<0.00001) and cast metal post (P=0.008). Conclusion: According to the results of this study, we can conclude that the selection of the removal technique will depend on the skill of the operator, and some loss of tooth structure will always occur, although greater loss is expected when only diamond burs are used for this purpose.(AU)

ABSTRACT

Many patients with large-area tooth defect need cast post-core crown restoration. However, the color defect of the cast post-core will affect the final restorative result, especially that of the anterior teeth. A new technology of color masking by applying CERAMAGE polymeric porcelain to the cast metal post-core surface improves the color of a full-ceramic restoration of anterior teeth and may provide a new alternative for the aesthetic repair of anterior teeth with a large area of defective tooth.

@#Introduction: To compare the anxiety levels demonstrated by children during cast removal procedure between oscillating saw vs cast shear methods. Material and methods: A randomised prospective study of 102 children (mean age 8.3 ± 3.5 years) with fractures involving upper or lower limbs. Children undergoing removal of cast were divided into 2 groups; either by an oscillating saw or a cast cutting shear. The level of anxiety was assessed by recording the heart rate with a portable fingertip pulse oximeter before, during and after removal of the cast. Objective assessment was performed by documenting the fear level on Children’s Fear Scale (CFS). Results: There was a significant increase in the heart rate of children during cast removal while using the oscillating saw compared to cast shear (p<0.05). The noise level produced by the saw exceeded 80 dB (mean 103.3 dB). The fear level was significantly lower in the cast shear group (p<0.05). Conclusion: The noise produced by the oscillating saw was associated with an increased anxiety level in children undergoing cast removal. Cast shear is a simple and inexpensive instrument that can be used for cast removal in overly anxious children.

ABSTRACT

Objective: To compare the reproduction trueness and precision of dental casts made by the conventional, milling and 3D printing techniques. Material and Methods: From an upper right side half-arch reference model (RM), 72 models were obtained and divided into three groups: conventional (CM), milled (MM) and printed (PM). All models were scanned and converted into standard tessellation language (.STL) files. The files were superimposed using 3D analysis software, and statistical analysis was performed using the root mean square (RMS) values obtained. The Shapiro-Wilk test was used to assess normality, and the Kruskal-Wallis test was used to compare groups (ρ < ⍺; ⍺ = 0.05). The Mann-Whitney U test was used for multiple comparisons among groups (ρ < ਕ; ਕ = 0.017). Results: There were significant differences in trueness (ρ = 0.000; ρ <0.001) and precision (ρ = 0.000; ρ < 0.001) among the three dental cast groups. Regarding trueness, CM presented better results, followed by MM and PM. Regarding precision, MM showed better results, followed by PM and CM, which did not show significant differences. Conclusions: For dental cast reproduction the conventional technique has the best trueness and the milling technique has the best precision.(AU)


Objetivo: Comparar a fidelidade e precisão de reprodução de modelos de trabalho pelas técnicas convencional, de fresagem e de impressão 3D. Material e Métodos: A partir de um modelo de referência (MR) de uma hemi-arcada superior direita, foram obtidos 72 modelos divididos em três grupos: convencionais (MC), fresados (MF) e impressos (MI). Todos foram digitalizados e convertidos sob a forma de ficheiros .STL (standard tesselation language). Os ficheiros foram sobrepostos utilizando um software de análise 3D, e através dos valores RMS (raiz do valor quadrático médio) obtidos foi realizada a análise estatística. Para avaliação da normalidade foi utilizado o teste Shapiro Wilk e para a comparação entre grupos foi utilizado o teste Kruskal-Wallis (ρ < ⍺; ⍺ = 0.05). Para as comparações múltiplas entre grupos, foi utilizado o teste U de Mann-Whitney (ρ < ਕ; ਕ = 0.017). Resultados:Existem diferenças significativas de fidelidade (ρ = 0.000; ρ <0.001) e precisão (ρ = 0.000; ρ < 0.001) entre os três grupos. Em relação à fidelidade, MC apresentou melhores resultados, seguido por MF e MI. Em relação à precisão, MF apresentou melhores resultados, seguido por MI e MC, que não apresentaram diferenças significativas entre si. Conclusão: Para reprodução de modelos de trabalho, a técnica convencional é a que apresenta maior fidelidade e a técnica de fresagem a que apresenta maior precisão.(AU)

ABSTRACT

One of the rare cases of the upper gastrointestinal tract obstruction is superior mesenteric artery syndrome (SMAS) but potentially life-threatening if not recognized early. It is due to loss of fat pad between the aorta and SMA which will lead to a compression of the third portion of the duodenum. It has a different angle which will lead to different presentations as well as severity. We are reporting the 15 year-old (y/o) female who is medically free, presented to the emergency department (ED) complaining of severe colicky epigastric abdominal pain for the last 5 days in the epigastric area. The diagnosis of SMAS was made after clinical and radiological investigation. After proper supportive resuscitative measures, definitive management of the surgery was done by laparoscopic approach (duodenojejunostomy). The diagnosis of SMA syndrome is considered challenging due to many presentations and might be confused with other clinical conditions. Unless early diagnosed and treated, the outcome might be catastrophic. Medical treatment is attempted first in many cases depending on the severity and presentation but if failed, surgery will be the best option.

Background: Among heart diseases, coronary artery disease is one of the major causes of death in developingcountries. Coronary circulation studied by various methods has demonstrated that there is a huge spectrum ofvariation in the branching pattern of the coronary arteries. Blood supply of the heart is achieved by two coronaryarteries originating from the ascending aorta. In present study we prepare casts of coronary arteries by usingcorrosive cast technique to study the origin, course, termination and branching pattern of right coronary arterywith its variations in human hearts.Materials and Methods: 30 fresh specimens of human hearts which were obtained from the post-mortem room,B.J.Medical College, Ahmedabad. The cannula with cone shaped tip was introduced into both the coronaryarteries through the coronary ostia. The lumen was cleaned and after that prepared resin was instilled incoronary ostia. After 24 hours the heart specimen was immersed in concentrated HCL. 30 casts of coronaryarteries were prepared and observed.Results: In All the specimens the right coronary artery ostium was present in the right anterior aortic sinus.Termination of RCA was found at the crux and beyond crux in 21 specimens (70 %); at the right border in 3specimens (10 %) and at the left border in 5 specimens (16.66 %). right marginal artery terminated at the apex in23 specimens (76.66 %) and near to the apex in 7 specimens (23.33 %). The PIVA arose from the RCA in 27specimens (90 %) and from the LCA in 3 specimens (10 %). In 25 specimens (83.33 %). the sinu-atrial nodal arterywas found to be originating from RCA and in 5 specimens (16.66 %) it originated from the LCADiscussion: Anatomical variations of the coronary arteries are important, as decisions with regard torevascularization procedures in many cardiovascular diseases depend on knowledge of the anatomy of eachindividual artery. Knowledge of the normal and variation in branching pattern of right coronary artery is helpfulto anatomist, radiologist and cardiologist as this is important for the interpretation of coronary angiography,various shunt surgery and stenting procedures.

Abstract Ultrasonic wave technology is widely used during dental treatments. We previously demonstrated that this method protects the gingival tissue. However, the physiological change on the gingival microvasculature caused by this method remains unclear. Objective The aim of this study was to investigate the relationship between the morphological and physiological effects on gingival microcirculation when preparing teeth, using the conventional dental turbine or ultrasonic method. Methodology The lower premolar teeth of beagle dogs were prepared along the gingival margin by using a dental turbine or ultrasonic wave instrument. Gingival vasculature changes were investigated using scanning electron microscopy for corrosion resin casts. Gingival blood flow at the preparation site was determined simultaneously by laser Doppler flowmetry. These assessments were performed immediately (Day 0), at 7 days and 30 days after tooth preparation. Results At day 0, in the turbine group, blood vessels were destroyed and some resin leaked. Furthermore, gingival blood flow at the site was significantly increased. In contrast, the ultrasonic group demonstrated nearly normal vasculature and gingival blood flow similar to the non-prepared group for 30 days after preparation. No significant alterations occurred in gingival circulation 30 days after either preparation; however, the turbine group revealed obvious morphological changes. Conclusions Based on multiple approach analyses, this study demonstrated that ultrasonic waves are useful for microvascular protection in tooth preparation. Compared with a dental turbine, ultrasonic wave instruments caused minimal damage to gingival microcirculation. Tooth preparation using ultrasonic wave instruments could be valuable for protecting periodontal tissue.

ABSTRACT

We report a 64 year-man with acute renal failure on top of hischronic renal disease due to diffuse diabeticglomerulosclerosis. Serum creatinine had reached 600umol/L from a baseline of 160 umol/L 4 months ago. Kidneybiopsy revealed diffuse broad casts obstructing the distaltubules with interstitial fibrosis associated with cast’sextravasation. The casts stained positive with kappa and notlambda indicating their monoclonal origin. The patient did nothave evidence of lymphoprolifertive disease and myeloma inparticular with normal skeletal survey and bone marrowtrephine biopsy. Moreover, serum protein and urineelectrophoresis failed to show monoclonal band. Only, directserum testing for free light chain confirmed high Kappa andnormal Lambda levels. The patient already had improved oncorticosteroids and subsequently Melphalan was added.Serum creatinine decreased to 180 umol/L by the end of this6-month treatment. The case illustrates a unique presentationof cast nephropathy without overt lymphoprolifertive disorderand with normal serum as well as urine proteinelectrophoresis. It emphasizes the need for kidney biopsyand/or direct measurement of serum free light chains fordefinitive diagnosis in such covert cases.

ABSTRACT

Membranous dysmenorrhea is a rare painful clinical condition associated with spontaneous expulsion of the endometrium as an entire piece, retaining the shape of the uterus. Authors report a case of membranous dysmenorrhea in a 36 year old multiparous woman, who was not on any hormonal therapy. She presented with history of menorrhagia for 20 days and severe dysmenorrhea for one day. During her second day of hospital admission, she expelled a fleshy mass resembling a decidual cast. Histopathological examination was consistent with diagnosis of membranous dysmenorrhea. The etiology of membranous dysmenorrhea is not very clear and hence reporting such rare cases may aid in understanding the etiology and pathophysiology of this rare condition.

Retention prevents dental restoration from being dislodged along its path of insertion. However, classic retentive factors are not always possible to implement in fixed prosthodontics. Therefore, it may be helpful to utilize non-conventional methods to enhance retention in situations where retention can’t be obtained from primary and secondary retentive factors. This article describes a technique to increase crown retention to cast post and core by using a horizontal retentive post to lock the crown to the cast post and core.

Resumo O CAST (Caries Assessment Spectrum and Treatment) é um instrumento desenvolvido para a detecção de cárie a ser utilizado em levantamentos epidemiológicos. Foi validado e tem se mostrado efetivo, fornecendo um diagnóstico mais preciso do estado de saúde bucal do que o critério OMS, recomendado pela Organização Mundial da Saúde. O objetivo deste artigo é comparar a apresentação dos resultados de cárie dentária utilizando o instrumento CAST e o critério OMS, numa mesma população. Foram avaliados por dois examinadores treinados na utilização do instrumento CAST 680 escolares de 6 a 8 anos do Distrito Federal, Brasil. A avaliação constou do índice de placa visível (IPV) e do índice de sangramento gengival (ISG). Os escores CAST dente foram convertidos em componentes ceo/CPO e calculados os ceod/CPOD. Os pais responderam a um questionário sociodemográfico. A idade média foi 7,45 anos (± 0,91). A prevalência de cárie na dentição decídua foi de 65,44% e 61,61%, considerando o CAST e o critério da OMS, respectivamente; na dentição permanente: 38,19% e 10,2%, respectivamente. A média do ceod foi de 2.4 (± 2.7) e a média do CPOD 0.16 (± 0.53). o IPV foi associado a maiores CAST máximos p < 0,005. O instrumento CAST demonstrou maior sensibilidade em identificar a presença e gravidade de lesões cariosas quando comparado ao critério OMS.


Abstract Caries Assessment Spectrum and Treatment (CAST) is an instrument developed for caries detection to be used in epidemiological surveys; it has been validated and is believed to provide a clearer picture of the oral health status than the criteria provided by the World Health Organization (WHO). This article aims to compare the epidemiological survey results using the CAST instrument and the WHO criteria in the same population. 680 schoolchildren aged 6-8 years from Federal District, Brazil, were evaluated by two examiners trained to use the CAST. The visible plaque index (VPI) and gingival bleeding index (GBI) were also evaluated. The maximum CAST codes per tooth were converted into the dmf/DMF, the mean scores for primary and permanent dentition were calculated. The mean age was 7.45(± 0.91) years. The prevalence of caries differed when CAST and the WHO criteria were applied. In the primary dentition, it was 65.44% and 61.61%, and for the permanent dentition, 38.19% and 10.2%, respectively. It was possible to calculate the mean dmft [2.4(± 2.7)] and the DMFT [0.16(± 0.53)] using CAST. VPI was associated with higher maximum CAST scores p < 0,005. The way CAST results are presented showed a higher sensibility to identify the presence and severity of carious lesions in comparison to the WHO criteria.

ABSTRACT

ABSTRACT Objetive. Characterize the genetic polymorphism type SNPs in the calpain (CAPN) and calpastatin (CAST) genes of Colombian creole hair sheep (OPC). Materials and methods. In 300 individuals belonging to two OPC subpopulations from the departments of Sucre (SC) and Valle del Cauca (VC) were genotyped by PCR-RFLP (MspI) for the CAST locus and by PCR-SSCP for the CAPN locus. The allelic and genotypic frequencies, the observed (Ho) and expected heterozygosity (He), the fixation index (F) and the deviations from the Hardy-Weinberg equilibrium (HWE) were calculated and a molecular analysis of variance to estimate the values of FST, FIS and FIT. Results. In the CAST locus, the MM genotype was the most frequent (83.9±1.1%), followed by the other genotypes (MN: 15.5±1.1, NN: 6.0±0.0%) and the allelic frequency of M (91.7±0.4%) exceeded that of N (8.3±0.4%). For the CAPN locus the heterozygous genotype (48.2±0.7%) was the most frequent, the other genotypes presented frequencies TT:44.7 ± 1.9 and CC:7.0 ± 1.4%. The T allele reached a frequency of 68.8±1.5% (C: 31.3±1.5%). Similar percentages of allelic and genotypic frequencies were found in the subpopulations. The He was less than the Ho in both loci, with negative values of F and deviations of EHW only in CAPN. All the variation found was due to differences within the individuals, with non-significant values (p>0.05) of FST, FIS, and FIT (0.002, -0.093 and -0.095, respectively). Conclusions. The loci studied has high variability, these results can be used for future gene-assisted selection plans to increase OPC productivity.


RESUMEN Objetivo. El propósito del presente estudio fue caracterizar el polimorfismo genético tipo SNPs en los genes calpaína (CAPN) y calpastatina (CAST) en el ovino de pelo criollo colombiano (OPC). Materiales y métodos. 300 individuos pertenecientes a dos subpoblaciones de OPC de los departamentos de Sucre (SC) y Valle del Cauca (VC) fueron genotipados por PCR-RFLP (MspI) para el locus CAST y por PCR-SSCP para el locus CAPN. Se calcularon las frecuencias genotípicas, alélicas, la heterocigocidad observada (Ho) y esperada (He), el índice de fijación (F), los desvíos del equilibrio de Hardy-Weinberg (EHW) y un análisis de varianza molecular para estimar los valores de FST, FIS y FIT. Resultados. En el locus CAST, el genotipo MM fue el más frecuente (83.9±1.1%), seguido por los otros genotipos (MN: 15.5±1.1; NN:6.0±0.0%) y la frecuencia alélica de M (91.7±0.4%) superó la del N (8.3±0.4%). Para el locus CAPN el genotipo heterocigoto (48.2±0.7%) fue el más frecuente; los otros genotipos presentaron frecuencias de TT:44.7±1.9 y CC:7.0±1.4%. El alelo T alcanzó una frecuencia de 68.8±1.5% (C:31.3±1.5%). Similares frecuencias alélicas y genotípicas se encontraron en las subpoblaciones. La He fue menor que la Ho en ambos loci, con valores negativos de F y desvíos de EHW solo en CAPN. Toda la variación encontrada fue debida a diferencias dentro de los individuos, con valores no significativos (p>0.05) de FST, FIS y FIT (0.002, -0.093 y -0.095, respectivamente). Conclusiones. Los loci estudiados tiene alta variabilidad, estos resultados pueden ser utilizados para futuros planes de selección asistida por genes para aumentar la productividad del OPC.

ABSTRACT

Decidual cast is the entire sloughed endometrium that takes the form of the endometrial cavity. It causes membranous dysmenorrhea because the intact cast passes through an undiluted cervix. It may be associated with ectopic pregnancy, incomplete abortion, non-pregnant state with use of progesterone, Depot medroxyprogesterone acetate (DMPA), rarely with oral contraceptive pills. Authors are reporting a case of recurrent decidual caste formation with membranous dysmenorrhoea in 33 years old women P3L3 who was on norethisterone acetate treatment for a typical uterine bleed (AUB). She presented with heavy menstrual bleeding with severe dysmenorrhea in Gynae causality of ESI Basaidarapur medical college, Delhi. She expelled decidual caste and required therapeutic Dilation and Curettage (D and C) to control bleeding per vaginum. Her histopathology report showed marked decidua like change of the stroma but no villi suggestive of endometrial caste.

INTRODUCTION@#Stable distal radius fractures in children are frequently treated by immobilisation with a cast and heal readily without complications. This randomised clinical trial aimed to assess patient satisfaction and casting-related clinical outcomes when using polyolefin cast, a new cast material, compared to the conventional fibreglass cast.@*METHODS@#A total of 80 patients (age range 7-16 years) with radiograph-confirmed stable distal radius fractures were recruited. They were randomised to either the fibreglass group or polyolefin group, with short arm cast immobilisation for 20-30 days. After cast removal, the incidence of skin rash, growth of hair and cast breakage was recorded along with the administration of patient satisfaction questionnaires. Mann-Whitney U test or Fisher's exact test was applied to compare results.@*RESULTS@#Overall, 34 patients from the fibreglass group and 31 patients from the polyolefin group were included in the final analysis. Significantly fewer patients from the polyolefin group reported itchiness during the casting period (p = 0.038). However, significantly more cast breakages were observed for the polyolefin group in the palmar bar region (p = 0.009). Patients from the polyolefin group were overall more satisfied (fibreglass group = 3.15/5 vs. polyolefin group = 3.74/5; p = 0.002).@*CONCLUSION@#Polyolefin cast reduces itchiness during casting and provides higher overall patient satisfaction during the treatment of stable distal radius fractures in children in tropical climates. However, patients should be counselled regarding potential cast breakage, which did not compromise safety, and the higher costs involved.

ABSTRACT

Objective To investigate the clinicopathological characteristics of renal light chain deposition disease coexisted with cast nephropathy (LCDD&LCN).Methods Patients with LCDD&LCN (n=10),isolated LCDD (I-LCDD,n=21) and isolated LCN(I-LCN,n=17) diagnosed byrenal biopsy in Peking University First Hospital from January 1,2000 to March 31,2018 were enrolled,and all cases were examined by light microscopy,immunofluorescence (IF) (including light chain) and electron microscopy (EM).The semi-quantitative evaluation of the main features of renal pathology was performed.The clinical manifestations and pathological features were reviewed and compared.Results LCDD&LCN was more prevalent in middle-aged males.Nine patients showed acute renal insufficiency with small molecular proteinuria (97.1%) and microscopic hematuria.The hematologic diseases included 9 patients of multiple myeloma.The type of monoclonal light chain in serum and urine by immunofixation electrophoresis showed λ dominant (5/8).By light microscopy,glomerular lesions presented with mild mesangial proliferation in most patients,and only one of them displayed mesangial nodular sclerosis.At the same time,acute tubular injury with light chain casts was the prominent feature,and the clinical manifestations and histological features of LCDD&LCN were similar to that of I-LCN.IF revealed linear staining of monoclonal light chain along the glomerular basement membrane (GBM),tubular basement membrane (TBM) and Bowman's capsule,and also positive in tubular casts.By electron microscopy,diffuse powder-like or granular electron-dense deposits located in the inner side of the GBM,the outer layer of the TBM,renal interstitium and arteriolar walls were observed.Conclusions Patients with LCDD&LCN manifest as acute renal insufficiency,and the majority have multiple myeloma.The pathology of LCDD&LCN possesses the features of both I-LCDD and I-LCN.The IF stain of light chains(κ,λ) and ultrastructural examination by electron microscopy are the inevitable methods for the diagnosis of LCDD&LCN.

ABSTRACT

PURPOSE: To compare the efficacy between cast immobilization and functional treatment using an ankle brace as a treatment for acute lateral ankle sprain. MATERIALS AND METHODS: This study reviewed the medical records of 157 acute ankle sprain patients who were treated between 2009 and 2014. A total of 101 cases were included in this study except for cases with a combined injury, and could not be followed up for eight weeks after the first visit. The patients were divided according to the treatment modality: a cast immobilization group (64 cases) and functional treatment group (37 cases). The clinical outcomes were assessed retrospectively based on the medical records of each group. The residual symptoms, such as pain, swelling, and instability, at three weeks after the primary treatment and at the last visit were compared. RESULTS: The residual pain and instability were significantly common in the functional treatment group at three weeks. Five cases (7.8%) of pain and one case (1.6%) of instability were in the cast group whereas nine cases (24.3%) of pain and six cases (16.2%) of instability in functional treatment group (p=0.021, p=0.014). On the other hand, there was no meaningful difference at the last follow-up. Residual pain, swelling, and instability at the last visit were noted in three (4.7%), six (9.4%), and four cases (6.3%) in the cast group, and three (8.1%), three (8.1%), and three (8.1%) were observed in the functional treatment group. Six patients refused cast immobilization. CONCLUSION: Although there was no significant difference at the last follow-up, cast immobilization appears to be more effective than a functional brace in terms of early pain relief and early restoration of ankle stability as a treatment for acute ankle lateral sprain in this study.
